The Patient Portal is a way for you to communicate with our office, request appointments, and securely view parts of your medical record on-line.

In order to use this feature, you will need to contact the office by phone or fax and provide your name, date of birth, and an email address for us to initiate your secure access to the Patient Portal. We will then give you your patient ACCOUNT number, which you will need to log-in and create an account.
